What are the requirements to avail your memorial services?

There are various requirements for each step of the memorial service process.

In order for a deceased to be retrieved by St. Peter Chapels from the place of death, these documents have to be presented:

  1. If the deceased is to be retrieved from the hospital:
    1. Temporary Death Certificate signed by the attending physician 
    2. Hospital release papers 
    3. Confirmation and Waiver signed by the family that the deceased is not COVID+  OR Swab Test / RT-PCR Result (if deceased has undergone the test)
  2. If the deceased is to be retrieved at residence:
    1. Barangay Certificate that the deceased is not on the watchlist for COVID-19 and is not a Suspect and/or Probable COVID-19 Case

In order for a deceased to be embalmed, these documents have to be presented:

  1. Death Certificate signed by the attending physician or the Barangay Health Officer confirming the cause of death
  2. In absence of a death certificate, a Certificate to Embalm signed by the hospital or Barangay Health Officer

In case there is a need to transport the remains, the following documents have to be secured from the local government office of origin:

  1. Certified True Copy of Death Certificate signed by Physician and Licensed Embalmer
  2. Transfer Permit
  3. Quarantine Permit (if transporting the remains via air or sea travel)
