Where can we get the Certificate of Cremation?

To process the certificate, kindly submit the following to your contracting St. Peter Chapel where the cremation was held. Our Chapel Manager and staff will be glad to assist you.

1.  Request letter stating the purpose of request.
2.  Copy of the memorial service contract made during the service.
3.  Valid ID of the contracting party for our reference and verification of relationship.
