How can I request for replacement of my lost Certificate of Full Payment (COFP)?

There are two ways to request for replacement of your lost Certificate of Full Payment (COFP).

If the plan was purchased through Sales Agent:

  • You may request for replacement of your Certificate of Full Payment.
  • Kindly submit a notarized affidavit of loss together with any government-issued ID at any St Peter Life Plan Branch nearest to you. 
  • You will also have to pay a replacement fee of P250.

If the plan was purchased through the St. Peter eStore:

You may submit your request for the replacement online through your St. Peter eStore account.

  • Just log in with your account details at www.online.stpeter.com.ph and click on the CERTIFICATE OF FULL PAYMENT TAB to begin your application. 
  • You will be required to upload a notarized Affidavit of Loss. After uploading the file you may now proceed with the payment for the replacement of P250.00. 
  • Your new CERTIFICATE OF FULL PAYMENT will be available on your St. Peter online account within seven (7) business days from the date of request. You may print out your COFP for your personal records only once. Should you lose the printout you will have to request a replacement.
