How can I reinstate my plan?

For eStore availed Plan, please Login and go to Reinstate my Plan. Click the contract you want to Reinstate.

For Branch availed Plan, please submit the following requirements to any branch near your location.

Reinstatement Requirements

  • Duly accomplished Reinstatement Form (RI)
  • Photocopy of one (1) valid ID
  • Life Plan Agreement (LPA)
  • Duly Accomplished Updating of contact details Form
  • Payments for Reinstatement Fee (500) and Monthly installment.

For plans bought before January 6, 2016:

You have two options to reinstate your account.

  • Option 1: Pay all unpaid installments based on the old price and surcharges plus a reinstatement fee of P500
  • Option 2: Pay at least 1-month installment based on the new price amount of your plan plus a reinstatement fee of P500

For plans bought after January 5, 2016 onward:

  • You must pay the prevailing installment amount of your plan plus the reinstatement fee of P500 to reinstate your account.

Please take note that once your plan is reinstated, the plan cannot be transferred,  or used to avail of memorial services within  30 days from the date of reinstatement. The plan will be subject to a 1-year contestability period from the date of reinstatement if the planholder is within the insurable ages of 18 to 60 years old at the time of purchase of the plan and prior to age 65 at the time of reinstatement.
