How can I change my personal information (Insurability) in your records?
Change of personal information can be requested at any St. Peter Life Plan Branch or the St. Peter eStore.
If the plan was purchased through Sales Agent:
To process the correction of your personal information, please visit our St. Peter Life Plan branch nearest to your location Mondays thru Saturdays from 8am to 5pm. Our Branch Manager and staff will be glad to assist you.
Kindly bring the following for our reference on your data correction request.
Change of Insurability
⁃ Signed Change of Insurability Form (To be provided by the branch)
⁃ Valid Government-issued ID (photocopy or scanned copy)
⁃ Scanned Copy of Birth Certificate
⁃ Certificate of full payment (if with issued Certificate already)
⁃ ₱50 processing fee (if without Certificate)
⁃ ₱250 processing fee (if with Certificate of full payment already)
If a plan is acquired through the St. Peter eStore:
Please note that the insurability of your plan is based on the Health Declaration you accomplished under Part III of your contract.
If you need to request corrections, kindly submit your request through our Contact Us Page and attach the following document(s):

Valid ID of the planholder

Please note that INSURABLE PLANHOLDER should be,
- between 18 years and 60 years old (not beyond 60th birthday)
- possess sound health and able to perform the normal activities in pursuit of livelihood
- have not consulted any physician for heart condition, hypertension, cancer, diabetes, lungs, kidneys or intestinal disorder, tuberculosis, or any other physical impairment nor have been confined in a hospital/clinic and received any medical or surgical attention.
Planholder must clearly understand and agree that the issuance of the Life Plan and the corresponding benefits are based on the truth of above-stated representations. Any false statements as regards to Planholders age or health as above-stated may render the Life Plan Contract rescissible.
