How can I change my personal information (Beneficiary) in your records?

Change of personal information can be requested at any St. Peter Life Plan Branch or the St. Peter eStore.

If the plan was purchased through Sales Agent:

To process the correction of your personal information, please visit our St. Peter Life Plan branch nearest to your location Mondays thru Saturdays from 8am to 5pm. Our Branch Manager and staff will be glad to assist you.

Kindly bring the following for our reference on your data correction request.

Change of Beneficiary/ies

• Change of Beneficiary Form (to be provided by the branch)

• Valid ID of the planholder

• Photocopy of the Life Plan Agreement contract

• Valid ID or Birth Certificate of the new beneficiary/ies

• Change Request Fee P50


If a plan is acquired through the St. Peter eStore:

Please login to your eStore portal and follow the procedures below to process the correction of your declared beneficiary/ies:

• Please login to your eStore portal

• Under Manage My Plan, click Update My Beneficiary

• Select the desired plan you wish to update

• Click (+) if you wish to add a beneficiary and (x) if you wish to delete a beneficiary

• Enter the necessary information, then click SAVE

• Once done, click PROCEED

• Evaluate the data, agree to The terms, and then click CONFIRM.

• Enter the OTP, then click SUBMIT

• Pay the Php 250.00 processing fee

• We will notify you via email on the status of your request.

 

IMPORTANT NOTE: For Legal Beneficiaries;

A. FOR MARRIED PLANHOLDERS -- Legal Spouse and Children

B. FOR SINGLE PLANHOLDERS (with children) -- Children

C. FOR SINGLE PLANHOLDERS (without children) -- Parents, Grandparents

Married but separated planholders (no final judgment from the courts declaring annulment of marriage, legal separation or judicial separation of community/conjugal property) fall under A.

Widowed planholders and those whose marriages are legally annulled/legally separated by final judgment from the courts with children fall under B and those without children fall under C.

Common-law spouses (live-in relationships), Friends and Strangers cannot be legal beneficiaries.
