How can I change my personal information (Address) in your records?
Change of personal information can be requested at any St. Peter Life Plan Branch or the St. Peter eStore.
If the plan was purchased through Sales Agent:
To process the correction of your personal information, please visit our St. Peter Life Plan branch nearest to your location Mondays thru Saturdays from 8am to 5pm. Our Branch Manager and staff will be glad to assist you.
Kindly bring the following for our reference on your data correction request.
Change of Residence Address
⁃ Updating od Contact Details Form (To be provided by the branch)
⁃ Valid ID reflecting your current address or Barangay Clearance/Certificate
⁃ Certificate of full payment (if with issued Certificate already)
⁃ ₱50 processing fee (if without Certificate)
⁃ ₱250 processing fee (if with Certificate of full payment already)
If a plan is acquired through the St. Peter eStore:
Please login to your eStore portal and follow the procedures below to process the correction of your address:
• Please login to your eStore portal
• Under Manage My Plan, click Update My Information
• Scroll down to your Residential Address
• Enter the necessary information then upload the necessary requirements.
⁃ Valid ID reflecting your current address or Barangay Clearance/Certificate
• Once done, click REQUEST FOR UPDATE
• Evaluate the data, agree to the terms, and then click CONFIRM.
• Enter the OTP, then SUBMIT
• Pay the Php 50.00 processing fee
• We will notify you via email on the status of your request.
