How can I change my personal information (Name) in your records?

Change of personal information can be requested at any St. Peter Life Plan Branch or the St. Peter eStore.

If the plan was purchased through Sales Agent:

To process the correction of your personal information, please visit our St. Peter Life Plan branch nearest to your location Mondays thru Saturdays from 8am to 5pm. Our Branch Manager and staff will be glad to assist you.

 

Kindly bring the following for our reference on your data correction request.

For Single Planholder

⁃ Data Correction Form (To be provided by the Branch)

⁃ Valid ID under correct name

⁃ Birth Certificate

⁃ Certificate of full payment (if with issued Certificate already)

⁃ ₱50 processing fee (if without Certificate)

⁃ ₱250 processing fee (if with Certificate of full payment already)

 

For Married Planholder

⁃ Data Correction Form (To be provided by the Branch)

⁃ Valid ID under correct name

⁃ Marriage Certificate

⁃ Certificate of full payment (if with issued Certificate already)

⁃ ₱50 processing fee (if without Certificate)

⁃ ₱250 processing fee (if with Certificate of full payment already)


If a plan is acquired through the St. Peter eStore:

Please login to your eStore portal and follow the procedures below to process the correction of your declared beneficiary/ies:

• Please login to your eStore portal

• Under Manage My Plan, click Update My Information

• Scroll down to the Personal Information

• Enter the necessary information then upload the necessary requirements.

 

For Single Planholder

⁃ Valid ID under correct name

⁃ Birth Certificate

 

For Married Planholder

⁃ Valid ID under correct name

⁃ Marriage Certificate

 

• Once done, click REQUEST FOR UPDATE

• Evaluate the data, agree to The terms, and then click CONFIRM.

• Enter the OTP, then SUBMIT

• Pay the Php 50.00 processing fee

• We will notify you via email on the status of your request.
