How can I change my personal information in your records?

Change of personal information can be requested at any St. Peter Life Plan Branch or the St. Peter eStore.

If the plan was purchased through Sales Agent:

You may apply for the correction of your personal information at any St. Peter Life Plan branch nearest to your location. 

Kindly bring the following for our reference on your data correction request.

(Change in Residence Address)

  • Valid ID bearing New Residence 
  • Photocopy of Barangay Certificate (Change to Residence Address)
  • Change Request Fee P50

(Change from Maiden Name to Married Name)

  • Photocopy of Marriage Certificate 
  • Change Request Fee P50

(Married to Widow/er)

  • Photocopy of Spouse Death Certificate 
  • Change Request Fee P50

(Married to Divorced/Annulled)

  • Any of the following documents: Annotated Marriage Certificate, Court Decree of Annulment and Declaration of Nullity of Marriage
  • Change Request Fee P50

(Change in name due to incorrect spelling)

  • Photocopy of Birth Certificate
  • Any government-issued valid ID
  • Change Request Fee P50

(Change in beneficiary)

  • Change of Beneficiary Form (to be provided by the branch)
  • Change Request Fee P50

If a plan is acquired through the St. Peter eStore:

You may change your personal information online through your St. Peter eStore account. 

  • Just log in with your account details at www.online.stpeter.com.ph and on your Account Settings click on EDIT to make changes to your personal information.
  • Please be ready to upload and submit scanned copies of your supporting documents and IDs.
  • Kindly make sure that you have the mobile phone that you used to register ready in order to receive the OTP required for the changes to take effect.
